Sliding Fee Program
Our Sliding Fee Program provides a schedule of discounts for services provided at Rambam Family Health for uninsured and underinsured patients. The amount of the discount is based on family/household size and annual income, and is updated annually using the poverty guidelines published by the U.S. Census Bureau. Depending on family size and income, discounts are available to patients at or below 200% of the Federal Poverty Level (FPL).
Patients at or below 100% of the Federal Poverty Level will be charged a nominal fee for services provided at Rambam Family Health. For more information, please call our office at

Not Insured?
If you do not have insurance, Rambam Family Health can help. Rambam’s patient advocates are available to provide insurance eligibility information and to enroll patients in government insurance programs and/or the New York State Health Exchange.
